As an Outreach Support Worker, you’ll be the bridge between people and the services they need, working alongside individuals with mental health support needs, often complex and enduring. You’ll work directly with individuals and families in their own homes, helping them navigate health and social care systems, access vital resources, and build the confidence to live more independently. Your support will have a lasting impact—improving wellbeing, reducing isolation, and fostering stronger, healthier communities.

The role is a full-time 37 hour a week contract and involves working 9-5.30 five days a week across a 7-day period (including weekends and bank holidays). However, we are happy to discuss flexible working, promoting work/life balance. The outreach service covers the Manchester Area with the expectation candidates would be willing to travel to support people in their own homes.

Role Responsibility

What You'll Do

  • Provide person-centred support to individuals in their homes and community settings.
  • Assist with daily living activities, from attending appointments to accessing community groups.
  • Offer emotional support and encouragement, helping people build resilience and confidence.
  • Work closely with healthcare professionals, social workers, and voluntary organisations to coordinate care.
  • Keep accurate records and contribute to care plans that reflect each person’s unique needs and goals.
  • Promote healthy lifestyle choices and connect people to wellbeing initiatives.

The Ideal Candidate

What You'll Bring

  • A genuine passion for helping others and improving community wellbeing.
  • Experience in a care, support, or community-based role (formal or informal).
  • Strong communication and listening skills, with the ability to build trust quickly.
  • An understanding of mental health, safeguarding, confidentiality, and professional boundaries.
  • Flexibility, empathy, and a problem-solving mindset.
  • A valid driving licence (desirable but not essential).
