We’re looking for an experienced Social Worker to join a Preparing for Adulthood team. You’ll support young people with additional needs and disabilities through the transition to adult services, helping them and their families plan for the future with confidence.

The role

What you’ll need
- A recognised Social Work qualification
- Current Social Work England registration
- Experience supporting young people with disabilities, additional needs or complex needs
- Knowledge of transition planning and preparing for adulthood pathways
- Strong assessment, care planning and partnership-working skills
- A person-centred, strengths-based approach to practice
